Mineral assessment questionaire
To check your mineral levels. Go to www.activeelements.com
Enter the following temporary username and password:

Then follow instructions. You will then be asked to add your own username, password and your details. (Please remember your username and password). The first question the system will ask you will be: "Is this your first assessment?" answer, "YES"
You will need to answer about 35 questions this will take you about 20 minutes. The information is secure and comes to Health Mechanics only. Please call so I can explain the results or I can post or email you a copy. Ph 4276094 or email info@healthmechanics.co.nz
Hint: At the very end of the questionnaire, you'll have the opportunity to enter any comments that you may feel are important or relevant to your case (the more you can tell us about yourself the better). Maybe give a bit of thought to these issues and make a few notes so that you're ready before you go online.
